Can I redeem a US card on a Pakistani Apple Account?

No. Apple checks the account's country or region. Changing it requires a zero balance, no active subscriptions and a payment method from the new country, so buy for the region the account already has.

Does the balance cover subscriptions?

Yes — Apple Music, Apple TV+, iCloud+, Apple Arcade and any App Store subscription can be paid from the Apple Account balance.
